Your First Appointment

During an initial visit, a detailed assessment of the client’s current situation and needs is conducted. The client discusses his or her reasons for seeking counseling, and the counselor asks questions about the client’s symptoms as well as any pertinent history that might contribute to the current situation. The counselor and client then work together to determine an immediate course of action.

Checklist

  • STEP 1: Prior to your first session, you will need to call the number for mental or behavioral health on the back of your insurance card.
    Ask your insurance representative the following questions:
    • Is my chosen therapist "in network" or "out of network?"
    • Is outpatient mental health counseling covered?
    • How many sessions will be covered? For how long?
    • Do I have a deductible that must be met before insurance will cover my session?
    • What is my financial responsibility, ie, copay or coinsurance–per session?
    • Do I need pre-authorization before my first session? (If so, you must obtain an authorization number and bring it to your first intake session. Without mandatory authorization your insurance can deny coverage of your session(s). This would mean you would be responsible for the full payment.)
  • STEP 2: Once you have obtained your insurance coverage and benefits information, please contact 512-248-8877 to schedule your initial appointment. Prior to attending your first session, we will fax you a copy of the paperwork to complete and bring to your first scheduled session.
  • STEP 3: We also need a copy of the front and back of your insurance card, as well as the following information:  
    Insurance Name
    Insurance Id#
    Primary Group #
    Phone Number on the back of card for Providers
    Phone Number on the back of card for Mental or Behavioral Health Services
    Primary insured name
    Primary Insured date of birth
    Primary Insured Social Security number
    Client’s name
    Client’s date of birth
    Client’s social security
    Home Address
    Home Phone number
    faxed to our secure fax at 512-248-2877 at least 48 hours prior to your appointment in order to ensure coverage. This information is maintained in a confidential manner and is locked in a secure facility. Due to HIPAA laws, insurance companies ask the following information of providers when we obtain coverage and benefits information.
